Health Screening DOE Buildings

Complete the Health Screener every morning BEFORE you enter the school building! https://healthscreening.schools.nyc/

Building Safety Protocols

  • All persons entering the school MUST fill out the Health Screening form every morning

  • Masks must be worn at all times (unless eating)

  • 3ft Physical Distancing will be maintained as much as possible

  • Every school will have ten percent of unvaccinated individuals who have submitted consent for testing in their school population tested biweekly. Students and staff who are fully vaccinated are not required to be tested. Please complete Covid Testing Consent Form
